New patient paperwork can be completed in the office, or we can mail it to you ahead of your visit — just ask when you call.
What to bring to your first visit
- Photo ID and your insurance card
- A list of your current medications and dosages
- Any prior vascular or ultrasound reports
- Compression stockings you currently wear, if any
- Your referral, if your plan requires one
